Below is the form for you to fill out. Please provide whatever information you think will help us design your puzzle - help us see your vision! Please describe what you envision & your vibe - it will help us get to know you and to design the puzzle that is right for you!

This is also where you will give us the locations that you want on the map that will be on your puzzle. The easiest way to do this is to use Google Maps:

> Find the establishment in Google Maps.

> Copy the name of the establishment and paste it into the box for the name of the location. You can then edit the name to be how you want it displayed on the map. E.g. Google Maps calls it 'Palace of Westminster' but you want it called 'Westminster Palace'.

This will prevent typos from showing up on your map.

If an establishment has more than one location with the same name, please also add the street address so we can map the right place!

Please attach up to 20 photographs to include on your puzzle. For best results send photos that are at least 1MB in size.
